What to Bring

Please arrive 15 minutes early to make sure your paperwork is complete.

Please bring with you:

An accurate list of medications and their dosages.
Your insurance cards.
All available medical records including blood work and prior cardiac testing.
It is helpful if you call your primary physician office to fax over lab work and pertinent information to aid in your evaluation. Our fax number is 845-268-0882.
