Insurance

Referrals: If your insurance plan requires a referral from your primary physician, please bring it with you to your appointment or have your physician fax it to us at: 845-268-0882

Claims: We participate with all major insurance carriers. Our billing department will be glad to submit the claims to your carrier. We will also obtain any pre-certification for procedures done in the hospital or office.

Co-pays: Will be collected at the time of your visit.

Non-participating plan: If you are insured by a plan that we don’t participate in, we will submit the claim form to your carrier, as a courtesy. However, you will be responsible for all charges incurred.
