Prescription Refills
Please keep in mind that prescription refills are for active patients with current blood work. This is for your safety!
Request a refill by calling our office at 845-268-0880. Or, call your pharmacy, and they can contact our office.
Don’t be caught short! Try to call 3-5 days before your prescription runs out so both our office and the pharmacy can make sure you have the medicines you need, when you need them.
When calling for a refill, please have the following information handy:
- Name of caller
- Name of patient
- Date of birth
- Name of medication, dose of medication, and directions
- Pharmacy name and phone number
